Synergy HomeCare of Mid-Jersey specializes in providing quality, compassionate home care to seniors and non-seniors in their own comfortable, familiar home setting. We are looking to hire per diem Registered Nurses to assist with home assessments and the configuration of home care plans. We provide non-medical home health services to cities throughout an area that includes Monmouth, Mercer, Burlington, and Ocean Counties. Our office is currently staffing “personal care” cases throughout Mid-Jersey.
Requirements of the position:

Ability to communicate in a professional and effective manner with clients, clients’ families, and the care team
-
Strong organizational and time management skills with an emphasis on punctuality
-
Reliable transportation and a valid N.J. State Driver’s License (with proof of auto insurance)
-
Must possess honesty, composure, empathy, and compassion
-
Ability to speak, read, and write effectively in English (bilingual a plus)
-
Ability to assess geriatric patients in a home setting
-
Clean and active New Jersey RN licensure
-
Must have professional liability insurance (proof of insurance required)
-
Clean criminal background history
-
Proof of a current TB Test
-
Experience conducting non-medical home care assessments a plus but not necessarily required
Responsibilities of the position:
-
Implementation of home care plans based upon a patient’s diagnosis and physical condition
-
Coordinate care with the care plan team
-
Perform follow-up quality visits in order to assess necessary changes to a client’s plan of care
-
Report directly to the managerial staff and your appointed supervisor
Synergy HomeCare of Mid-Jersey offers flexible work schedules to accommodate your lifestyle. This is a per diem position based upon our day-to-day needs. Most assignments/cases require a visit to the client’s home of approximately 30 minutes and you will be compensated at a per case flat fee rate.
