As a Per Diem Evercare Nurse Practitioner you will perform proactive and acuity visits to patients in Toledo-area long term care facilities on weekends; covers for PTO as needed and available. Additionally, you will build meaningful relationships with the residents and their families.

- Working with primary care physicians to provide the best care possible.
- Collaborating with the nursing staff and the patients' families

- Current, unrestricted NP license the State of Ohio
- DEA License
- Nurse Practitioner national certification as ANP, FNP, or GNP required
- A clinical background in adult, family or geriatric specialties
- Past experience working in a nursing home or with seniors in other settings
- Intermediate level of proficiency in PC based word processing including Microsoft Word and Outlook
