Provides skilled nursing care to terminally ill patients according to a Plan of Care (POC) approved by the physician under the supervision of the Registered Nurse (RN).

Louisiana agencies are licensed and certified by the Louisiana Department of Health and Hospitals to provide home care and hospice services to eligible Medicare and Medicaid recipients
Job Requirements:
Graduate of an Accredited LPN School. Minimum of one (1) year clinical experience as a LPN in acute care setting. Hospice experience preferred. Current LVN license in Louisiana. CPR certification. Must have a current Louisiana drivers license with current automobile liability insurance.
