Serves as an assistant to the agency Administrator to assure accurate computer data entry, updated patient records, and oversight of daily agency office processes.

Home Health Team Leader Assistant-Home Health LPN


Job Responsibilities

 

Assists with phone calls from field staff, physician offices, referral sources, patients, and takes messages and orders as needed.
 

Frequently monitors the status of incoming faxes, report to appropriate personnel.

 

Maintains competency through assigned LHC Connect courses and required in-services.

 

Assists with phone messages and answers calls and questions in a timely manner.

 

Assists with visit verification, scheduling and projection of visits, and other reports in hospice computer system.

 

Performs high risk patient phone calls as assigned.

 

Assists with the Interdisciplinary Group.

 

Assists the Administrator with Misys weekly reports.

 

Completes any other duties as assigned.


Must have LPN license, detail oriented and have excellent computer skills.



Home Health Team Leader Assistant-Home Health LPN 


 

Experience Requirements:
Minimum of one year experience in home health or hospice care. 

License Requirements:
Current licensure as a licensed practical nurse in state of employment.
Current CPR, driver’s license, valid vehicle insurance, and access to a dependable vehicle. 

Skill Requirements:
Working knowledge of hospice computer system.
