Sentara Home Care is looking for a dynamic Home Care Coordinator for full time position in our Lakeridge Branch. This position will be primarily located at Sentara Potomac Hospital.
Some travel may be required.
The HCC will gather and interpret medical and financial date, meet with patients, families, caregivers and consult with clinical staff members and physicians to obtain referrals.
Job Requirements:
Knowledge of home health regulations, home care, discharge planning, case management is required.
One year of home care experience preferred.
Education:
Graduate of accredited school of nurising (RN) and current Virginia license
