Case Manager - 40 hours per week - 3 months
Provides management services to each patient ensuring a detailed and personalized plan of care focused on individual patient goals. Serves as liaison for patient and family with staff and overseeing day to day care to ensure optimum patient outcomes.
Must work well with interdisciplinary team with a patient-centered focus to ensure exceptional patient outcomes and discharge.
Requires strong work history with good references, good communication skills, and the desire to continuously obtain excellent patient outcomes.
